Our History… Our Legacy

Originally known as The Bronx Health Plan, Affinity Health Plan opened for business in early 1987 with a singular goal: to offer affordable, high quality health care coverage to needy New Yorkers by working closely with community health centers and other primary care practices. Affinity was among the first health plans licensed in New York State to serve public-sector health programs. It grew from a handful of members in 1987, to more than 300,000 by late 2021, when the Health Plan’s operations were sold.

In late 2021, Affinity Health Plan was renamed Affinity Legacy, Inc. Although it bears a new name, the organization’s mission endures: to improve the health and well-being of the people in the communities we serve.

To ensure this mission, Affinity Legacy is providing one-time grants to eligible community based organizations that operate programs focused on mental health, food insecurity, incarcerated individuals and consumer health education and workforce advancement. The programs must serve residents within Affinity’s 10-county service area consisting of the Bronx, Kings, Nassau, New York, Orange, Queens, Richmond, Rockland, Suffolk and Westchester.

It is our privilege and most cherished goal to continue the work started decades ago and leave a legacy of well-being. We truly hope that our current efforts will create healthier, happier communities far into the future.
